Managing chronic and complex conditions is challenging. So we've developed Chronic Disease Management programs** designed to help members meet the challenges of
Diabetes, Asthma, and
Congestive Heart Failure.
This is done through:
- Information sharing
- Warning signs and symptom identification
- Treatment explanations and encouragement
- Free resources from external sources
- Free or discounted medical supplies
These programs help our members take a more active role in maintaining and improving their health. Through education and awareness, our disease management programs can help reduce emergency room visits and hospital admissions.
Chronic Disease Management Programs are voluntary and available at no cost to eligible members.
**Access to these programs is determined by the health plan selected. Please remember that all decisions that require or pertain to independent professional medical or clinical judgment or training, or the need for medical services, are solely your responsibility and that of your treating Physician and health care Providers. You and your Physicians are responsible for deciding what medical care should be rendered or received, and when that care should be provided.
