Frequently Asked Questions to Become a Participating Provider



NOTE: Physicians and providers are selected to participate in our networks based on an assessment and determination of the network's needs. Completion and submission of the application and the required documentation does not guarantee inclusion in any of Blue Cross and Blue Shield of Florida's networks.


How do I become a Blue Cross and Blue Shield of Florida participating provider?

To be considered for participation in our networks, you will need:

1.

An assigned Blue Cross and Blue Shield of Florida (BCBSF) provider number. If you do not have one, please call (800) 727-2227 and say "More Choices", then "Provider Number" to request an application or you may also complete the Provider Registration Form.

If you already have a number, proceed to step two (2).

Send us a letter indicating you are interested in participating in our network. This letter must be signed by the provider or facility representative and include:

  • BCBSF provider number
  • National Provider Identifier (NPI)
  • Council for Affordable Quality Healthcare (CAQH) number - You may use this hyperlink to access their website
  • Tax Identification Number (TIN) - include a W-9 form
  • Full legal name
  • Network(s) in which you are requesting participation, based on the following products
    • Insurance Based - PPO and Traditional
    • HMO
    • Medicare Advantage
  • The physical address of your practice and the billing address, if different than the physical address
  • Fax the letter to Network Management at (904) 997-5716
  • For ancillary facilities/providers:
    • Type of provider (i.e., Dialysis Center, Durable Medical Equipment, Skilled Nursing Facility, etc.)
    • Certifications
    • Status of practice - new practice, taking over existing or changes to existing practice
  • For physicians:
    • Physician specialty, including board status
    • Any physician group affiliations including the BCBSF assigned P.A. group number
    • Hospital privileges, if applicable
    • Please indicate if you are a Hospital Based Physician (HBP)
    • If you are an HBP, who also practices outside the hospital setting, please list the additional settings (i.e., ambulatory surgery center, pain clinic or other specialty clinics, etc.)

What happens after Blue Cross and Blue Shield of Florida receives my letter?

Network staff will assess your letter of interest and determine network need for your provider type or specialty. You will receive a written response within 60 days.

How can I check on the status of my request?

If at any time you have questions about the application or want to check on the status of your application, you may call Network Management at (800) 727-2227 and say "More Choices", then "Network Management".

Why does Blue Cross and Blue Shield of Florida require credentialing to join their network(s)?

The verification of credentials is an integral part of Blue Cross and Blue Shield of Florida's network process. It helps ensure our members have access to quality care and it is also required to meet both state and federal guidelines.
